Priorities:

Purpose:

To help reduce disparities and improve older adult health outcomes across racial/ethnic groups through outcome identification, tracking, and reporting.

Responsibilities:

  • Identify potential areas for improvement of the health and wellbeing of older adults/caregivers

  • Develop valid, reliable outcome measures

  • Identify strategies to help reduce disparities through performance measurement

  • Develop an action plan - with short, mid, and long-term recommendations.

Work in partnership with the Los Angeles County Aging and Disabilities Department, the Department of Public Health, academic partners, and community stakeholders to support efforts to reduce health disparities.

Goals:

Short-term: Identify additional healthy aging/health disparities indicators and resources to address health disparities. Develop an evaluation framework.

Mid-term: Seek and secure funding to support dashboard launching, expansion, member training, and resources sharing. Seek and secure funding to support evaluation activities.

Long-term: Provide training in the use and utilization of the Health Aging Dashboard. Identify and evaluate three innovative strategies/interventions. Conduct bi-annual LAACHA New Direction Member Survey.

Los Angeles Healthy Aging and Chronic Disease Data Dashboard

The Los Angeles Health Aging and Chronic Disease Data Dashboard summarizes data on the quality of life of older adults across Los Angeles County.  The dashboard also provides data to support community members in applying for their own respective grants and creating innovative programs aimed to improve the lives of older adults in Los Angeles County.

The Outcomes & Evaluations Committee meets Ad-hoc to discuss ways available data can support LAACHA members, reduce health & racial disparities, and other committees’ needs.
